Abstract
Fiberglass reinforced plastics (FRP) tanks are designed and fabricated based on industrial standard that is agreed both manufacturer and user. The four 50 cubic metre FRP chemical tanks on a multinational Indonesian company in west region of Java were supplied by a national fiberglass manufacturer and have been installed since 1999. One of the four tanks failed on July 2008. The bottom cover of the tank broke because the unstable of the concrete foundation so that the chemical fluid (chloride acid) went out. The broken tank was repaired with the same resin by hand lay up. On August 2008, the tank failed again on the top cover. The negative (vacuum) pressure by hydrostatic test caused the top cover of the tank deflected. The relaxation of the internal pressure until atmosphere pressure produced the spring force on the top cover to the normal position, so that many accessories on the top cover, i.e manhole, flanges etc. cracked or broke out from the top cover.
