One of Universitas Terbuka (UT) vision is to become one of centers of excellence among distance education institutions in Asia in 2010. As one of strategic steps to accomplish this vision, UT develops and implements quality assurance (QA) system. The system requires its unit, both in central offices and regional offices, to develop and send report on implementation of the QA in their respective units twice a year. The report has to follow specific formats prepared by the Centre for QA. The format, known as Format of Activity Status, consists of detail indicators (time, personnel, and resource) required in the activity. This article analyzes quality of reports on registration and examination activities submitted in 2004 by UT regional offices. The analysis includes degree of completeness of the reports, correctness use of the formats, and accuracy of data reported.
