Obesity is a pathologic state charactrerized by an excessive accumulation of fat that maybe generalized or localized. Insulin sensitivity decreases in obesity and it is accompanird by hyperinsulinemia, The mechanism linking obesity and decrease of insulun sensitivity is unknown. The level of circulating free fatty acids and fat oxidation seems to play an important role in the pathophysiology of insulin resistance in obesity. Hyperinsulinemia can increase mitosis of interstitial theca cells of the ovarian follicie. Over growth of the theca cells will enhance the process of aromatisation that changes androstenedione to periphal estron. The evelations of ovaroan androgen increase free ethinyledtradiol level. The elevation of periphal estron and free ethinylestradiol level enchances in appropriate levels of Luteinizing hormone and the reduction of Follicie Stimulating hormone levels. These conditions cause an ovulation and the development of infertility.
