Quality, recently becomes a mandatory. Quality considers as a mean of survival (competitive advantage) among industries. Most of industries pursue quality improvement programs/project within their products, processes, or services they offer to win the competition. Unfortunately, a result of a survey tells us that only a few of them were successful in their quality improvement program. One of the reasons might be the planning process that is not well performed, eventhough there are some management tools for quality improvement planning such as Shewhart PDCA Cycle, Deming PDSA Cycle, Japanese Kaizen, New Seven Tools, Shiego Singo Poka Yoke, DMAIC Six Sigma Breakthrough Road Map, Phillip MEDIC Cycle, or even the Besterfield Seven Steps. There is an hypothesis that the reason of the poor planning of the quality improvement process is the difficulties of understanding the above management tools, so that the employee as the edge of the quality improvement program can not proceed step by step activities accurately. Generally the quality improvement tools can be categorized into 4 (four) group of activities i.e. (1) The activity group for identifying problems, (2) The activity group for determining the cause of the problems, (3) The activity group for planning the quality improvements, and (4) The activity group for performing the quality improvement planned. In line with this category the researcher designed a new quality improvement cycle, Indonesian version, called PETA cycle (stand for Potret = picturise, Evaluasi = evaluate, Terapi = treat, Aplikasi = application).
