Isi artikelThe initial report of bisphosphonate therapy in children was nearly 30 years ago. Despite widespread use in adults, the pediatric experience with this class of drugs has been limited. Although recent experience suggests that concerns about adverse effects on the growing skeleton are exaggerated, particularly with the newer compounds, none of these drugs are labeled for use in children. It is incumbent upon clinicians contemplating the prescription of a bisphosphonate for a child to inform family members of this limited experience. All bisphosphonates are synthetic analogs of pyrophosphate, in which the oxygen of the phosphoanhydride bond (P-O-P) is replaced by a carbon, forming a methylene bridge (P-C-P). This substitution allows the addition of side chains to the methylene carbon, which confers to each molecule subtle yet unique physicochemical and biologic properties. Such biochemical manipulation has resulted in progressively more potent antiresorptive analogs, which may be classified into 3 generations.
