Health care reform is defined as the process that seeks major changes in national policies, programs and practices through changes in health sector priority, laws, regulations, organizational structure and financing arrangements. The central goals are most often to improve access, equity, quality, efficiency and / or sustainability. In indonesia the health care reforms refer to national health system. So based on those, health care reforms must entail major changes namely : a. encourage and enhanc eprimary health care as leading priority both personal and community health care b. legitimately national development (economic, agriculture, etc) should be based on health view. Health sector ideally should be among government's priority. Inter - relationship with other development sector is a must c. shift paradigm from desease paradigm to health paradigm orientation as strategy and policy inhealth care d. health financing systems should use social health insurance, where premium for the poor is funded by government in order to create equity and acces e. decentralization and empowerment toward health urban / district with central orbit at the province level.
