Resection is the only chance for cure for patients with pancreatic cancer. The extent of pancreatic, gastroduodenal, and lymph node resection, however, remains a matter of debate. As the morbidity and mortality rates of pancreatoduodenectomy have decreased, more extensive resections, such as regional pancreatectomy, have become more feasible options. Nevertheless, tumor biology and adjuvant therapy are the most important determinants of ultimate outcome. Therefore, less extensive procedures (such as the pyiorus-preserving pancreatoduodenectomy), which are associated with shorter operative time, reduced blood loss, and equally long but better quality of survival, are believed by many surgeons to be the optimal type of resection.
