Postsurgical adjuvant chemotherapy has been shown to reduce recurrence rates and prolong survival in selected patients with colon cancer. Chemotherapy with pelvic irradiation has been advocated postoperatively for stage II or III rectal cancer patients. Results of recent adjuvant therapy trials in colon and rectal cancer have provided insight into the selection of patients, treatment duration, and the optimal chemotherapy regimen. Future trials will examine new agents effective in advanced disease and monoclonal antibodies that may have selective activity in minimal disease. In addition to overall survival, these trials will examine quality of life and pharmacoeconomic issues.
