A variety of techniques are now available to treat distal rectal cancers successfully and allow for preservation of the anus. This article reviews the surgical technique and results of local excision, local excision with adjuvant therapy, and proctectomy with coloanal anastomosis. These sphincter-sparing techniques, along with other therapies (i.e., chemorad iation), appear to give equivalent outcomes compared to abdorn inoperineal resection for selected cancers. The overall effect on function and quality of life needs to be studied further to measure more accurately the total effect of these sphincter-sparing procedures on the patient’s lifestyle.
