Transaortic mesenteric endarterectomy and antegrade aortomesenteric bypass offer advantages over other methods of treatment for mesenteric artery occlusion secondary to atherosclerosis, They originate from minimally diseased or endarterectomized aorta, avoid problems of alignment or compression encountered with other techniques, and provide an autogenous alternative for multivesse] reconstruction Although antegrade bypass is more versatile and more widely applicable, transaortic endarterectomy lends itself to simultaneous correction of clinically significant renal artery disease. Regardless of the method of mesenteric reconstruction the authors advocate intraoperative completion duplex sonography to assess the technical adequacy of mesenteric artery repair.
