Duplex ultrasonography emerged as a non invasive diagnostic aid for splanchnic artery stenosis in the early 1980s, until which time arteriography had been the only objective clinical test available. Initially, investigators established a working knowledge of the duplex ultrasonography characteristics of normal splanchnic vessels, including responses to various physiologic and pharmacologic manipulations. This led to the development of duplex blood flow velocity criteria to identify more than 70% stenosis of the superior mesenteric artery. Although duplex ultrasonography alone is not a diagnostic indicator of mesenteric ischemia, it has become a first-line study for the evaluation of splanchnic artery stenosis in most cases.
