The incidence of Barrett’s esophagus and its malignant degeneration to adenocarcinoma is increasing in the western world. Medical therapy with proton-pump inhibition and surgical antireflux procedures are competitive, but neither guarantees protection against malignancy. Endoscopic biopsy and surveillance programs are necessary to make an early diagnosis of dysplasia or cancer. Ablative therapy combined with antireflux measures are being tested in several centers.
