The ileal pouch—anal anastomosis has gained a primary role in the surgical treatment of chronic ulcerative colitis. The rationale for this operation is that patients are cured of their disease and a permanent abdominal stoma is avoided. Technical modifications in the operation have focused on restoring reservoir capacity and preserving anal sphincter function, thus facilitating fecal continence. The physiologic changes following ileal pouch—anal anastomosis remain incompletely understood. Most patients, however, lead a normal lifestyle and are satisfied with the operation.
