The diagnosis of inflammatory bowel disease is established by the documentation that intestinal inflammation is the cause of the patient’s presenting symptoms. SimultaneouslY, it is nec¬essary to assess the patient’s need for resuscitation and to undertake systemic evaluation of specific etiologic agents for intestinal inflammation. If no specific cause is found, Crohrt’s disease and ulcerative colitis become the most likely diagnoseS~ and specific tests are indicated to help differentiate these two diseases and thus choose optimal therapy. In the follow-up of patients with Crohn’s disease and ulcerative colitis, disease severity will need to be assessed periodically. Complications of both these illnesses are less likely to prove fatal if diagnosed early.
