The presence of urinary tract calculi during pregnancy is a dramatic and potentially serious problem for the fetus and the mother alike. The anatomic and physiologic changes in pregnancy add to the diagnostic and therapeutic challenge. A high degree of alertness is needed to the possibility that colic, flank pain, and other symptoms as well as problems with labor are caused by stones. Renal ultrasonography, the most appropriate first-line test, may by itself provide the diagnosis. Radiologic studies, however, including excretory urography, also may be needed. All but a few of the forms of treatment used in the nonpregnant population are applicable in pregnant patients as well.
