Ovarian cancer presenting in pregnancy is, fortunately, a rare event with an overall incidence of 1 in 5000 to 1 in 18,000 live births. Of adnexal masses persisting into the third trimester, however, approximately 2% to 5% will be malignant. Proper intragestation management of adnexal masses, including imaging, surgical management, and potential adjuvant cytotoxic chemotherapy, is crucial for optimal outcome.
