Gastrointestinal vascular malformations are responsible for acute lower gastrointestinal bleeding in as many as one third of patients more than 60 years old, and in 2% to 5% of upper gastrointestinal bleeding episodes. Diagnosis is confirmed at endoscopy, and vascular malformations can then be coagulated with the use of a number of thermal coagulating devices. The argon laser, heater probe unit, and the BICAP bipolar electrocautery system are effective and safe throughout the gastrointestinal tract. Mono-polar electrocautery and the Nd:YAG laser are equally effective, but their greater and less predictable depth of coagulation increases the risk of perforation when they are used in the cecum and the right colon.
