Gastric reduction operations were developed because of the unavailability of an effective nonsurgical treatment for a morbid and lethal disease called severe obesity. Gastric bypass and vertical banded gastroplasty (VBG) are the two most frequently used operations today. Lessons from the last 25 years that decrease risk, reoperation rate, and unwanted side effects are reviewed. VBG is the operation of choice.
