Isi artikelAll gastric operations disturb gastric motility and emptying to some extent. The disturbance is great enough to result in a postgastrectomy syndrome in about 20% of operated patients. Clear definition of which motor abnormalities are present through the use of endoscopv, contrast radiography, and scmtigraphy is critical for the development of a rational treatment plan for the syndromes. Initial treatment usually consists of medical therapy and dietary and behavioral modification. Should these treatments fail, then operation is required. Construction of a Roux-en-Y gastrojejunostomy diverts pancreaticohiliarv secretions away from the gastric remnant in alkaline reflux gastritis and slows rapid upper gut transit in dumping. Resection of a dilated, flaccid stomach speeds gastric emptying in chronic gastric atony. About two thirds of patients with postgastrectomy syndromes are restored to good health by these reconstructive operations.
