The precise role of extended lymph node dissection in the surgical treatment of gastric adenocarcinoma is undefined. Although a proportion of the reported improvement in survival claimed by authors from Japan has been attributed to methodic gastric resection with extensive lymphadenectomy, the Japanese approach also includes an intensive screening program, rigorous histopathologic staging, and the aggressive use of adjuvant chemotherapy. The contribution of each of these components to improved survival is difficult to determine. In Western countries, extended lymph node dissection has not resulted in enhanced survival and has been associated with increased postoperative morbidity and mortality. Prospective trials that use a uniform staging system are needed to determine the role of radical lymph node dissection in the treatment of gastric adenocarcinoma.
