Duodenal ulcer disease is less prevalent now in the United States than it was a few decades ago. Improved medical therapy has decreased greatly the need for surgical control of chronic ulcer pain. Most operations are nmv performed for acute life-threat-ening ulcer complications. Proximal gastric vagotomy has emerged as the safest, least harmful, and best procedure for most patients with intractable pain or perforation and for selected patients with hemorrhage. Modification of the operation using laparoscopic techniques currently is under evaluation
