The two major problems of health service in Indonesia right now are the cost and the quality of the service. In order to deal with these problems the approach that can be taken is to conduct family health service, which one of the characteristic is to put forward an effective and efficient service, with special concern ofpromotive and preventive efforts. Family doctors act as a gate keeper in health service, supported by Public Health Maintenance Assurance (PHMA) system, providing the first level ofservice, thourough and sustainable with family approach. The role offamily doctors in health service in Indonesia is a need. This can be started by improving the education system in medical faculties in accordance with answering three major challenges; putting the post P17’ doctors to use, flourishing the PHMA and facing the globalization.
