The process of prescription writing and dispensing is governed by regulatory systems, the purpose of which is to maximize the safety and efficacy of the product supplied. Community pharmacists have an important role in assessing the prescriptions to ensure that they are appropriate to be dispensed. The aim of this research is to classify errors on prescription writing from general practices and to measure the frequency of these errors. It was designed by a retrospective analysis. The study was conducted on prescriptions from three pharmacies respectively which were near with general practice doctors, general hospital and residency. All prescriptions analyzed from the three pharmacies were those dispensed during April 2003. A total of 2445 prescribed items were analyzed and 204 errors were detected giving an error rate of 8,34% The most common errors were related to the incomplete direction for using the medicine with an error rate of 35,29%. While the less common errors were related to patient suffered from short supply of medicine due to special pack rules with an error rate of 0,49%. This phenomena should be warned by community pharmacists in order to imply and improve their role in pharmaceutical care practice. By the ability to recognize and identify those errors, pharmacist would be able to make appropriate decision in resolving those problems, so that the patient can achieve their expected therapeutic outcomes from their medicine.
