The recent discovery and characterization of the gene defects involved in several familial cholestatic diseases has led to improved understanding of the molecular mechanisms of cholestasis and bile formation. Our understanding of the process of cholestasis and bile formation are initially discussed. This leads into a description of the ontogeny of hepatobiliary function and how it relates to neonatal susceptibility to liver disease. Finally, a description of the known bile transporter defects in progressive familial intrahepatic cholestasis is provided to show the known mechanisms of cholestasis and how they may apply to neonatal liver disease.
