A needs analysis was undertaken on the Southern Illinois University-Carbondale (SIUC) campus during the 1997 spring semester to determine the language and communication problems that exist between the staff of the Student Health Program (SHP) and their international student patients. Sources of data in this study were: (a) 123 questionnaires completed by international students upon leaving the SHP and 100 questionnaires completed by the SHP staff members, (b) nominal group process feedback involving 7 SHP staff members and (c) field notes from on-site observations by the researcher. Research questions addressed were: (a) Do the international students feel they have problems in understanding the SHP staff and do the staff members perceive that they are having difficulties in being understood? (b) Do the international students perceive that they have difficulties in making themselves understood by the SHP staff and do the SHP staff members feel that they are having difficulties in understanding the international students? (c) How do the problem areas in communication mentioned by the international students correspond with those indicated by the staff? The term understand is analyzed in terms of (a) the ability to comprehend the actual speech patterns and styles, (b) the ability to understand the subject matter and vocabulary, (c) the ability to discern and negotiate the pragmatic aspects of the interactions and (d) the ability to grasp the medical systems and procedures. Results show that the students and staff agree on several problem communication areas including medical vocabulary and pragmatic problems due to different procedural and cultural expectations. Additionally, each group mentioned problem areas not addressed by the other such as prejudice, manner of speaking, appropriate feedback and the affective value of a polite, kind communicative approach.
