Here is a story of quality improvement in the Hydrofluoric Acid plant through use of Boolean algebra in fault tree analysis. The plant breakdowns were reduced significantly by the help of systematic approach in analyzing the problems. Through phenomenon analysis and FTA, the root causes were identified and corrective actions taken. The Boolean algebra was used to understand the combination of events which were leading to failure. Use of Boolean algebra helped to simplify the complex events into simple events which was easy to visualize and understand The success of solving this complex problem largely depends upon understanding of the process and effective use of FTA through phenomenon analysis
