The project's objective is decreasing premature termination of Continuous Renal Replacement Therapy (CRRT). The team adopted the Plan-Do-Check-Act method. Pre-implementation survey on using spiking method was done at Intensive Care Unit. Due to improper breaking of frangible seal in replacement bag thus causing dialysis machine to reflect incorrect pre- and post-weight value of patient. The team eradicated a number of CRRT premature termination and clotting related to “incorrect weight change” to almost zero. The project managed to save $16,584 for the department annually. Beside, saving cost, patient’s recovery outcome is expedited and improves with uninterrupted CRRT.
