Reducing the number of medical incidents is an important issue faced by hospital administrations. There have been incidents in which a medical staff member has mistaken one patient for another; such incidents are referred to as wrong patient incidents. Hospital administrations have tried to prevent these kinds of incidents, but have been unsuccessful. Furthermore, there has been no reduction in the number of these incidents. This is because medical staff members cannot understand the root cause of these incidents and are unaware of an appropriate method to analyze them. In this paper, we consider an analytical technique aimed at reducing such incidents.
