Isi artikelThe demand for nuclear power has been steadily increasing because of global warming due to power generation using fossil fuels. Preventive measures such as the prevention of recurrence are critical to ensuring nuclear safety. Root cause analysis (RCA) is an effective tool for the prevention of recurrence. In this study, we propose a procedure for RCA from the viewpoints of the business process and quality management system (QMS) in order to analyze the factors contributing to non-conformance more effectively. This procedure was applied to two actual accidents. As a result, it was determined that the procedure was useful to analyze the factors more effectively.
