As a result of advances in medical technology for serious diseases, the number of patients who need rehabilitation after treatment to recover the quality of life has been increased. Rehabilitation is provided based on a rehabilitation program. In this study, we aim to establish a model for designing rehabilitation pathway, which is a basis of the rehabilitation program. In this paper, we present a logical model to determine appropriate rehabilitation exercise for each patient. Second, we develop an implementation system based on the model. Third, we applied our model to hypothetical case in through workshop to confirm its validity.
