Isi artikelBackground: The resistance case is a problem for TB prevention and eradication programme in the world. Therefore, it is important to investigate the rate and the pattern of primary resistance in pulmonary tuberculosis. The aim of this study was to evaluate the incidence and pattern of primary resistance in category I of pulmonary TB patients in H. Adam Malik Hospital-Medan, Indonesia. Methods: This is a retrospective cross sectional descriptive study. Data retrieved from medical records with a time span of October 2010 until July 2011. Subjects were patients with no history of antituberculosis drugs treatment or ever consumed less than 1 month. Conducted direct smear examination of sputum, cultur and sensitivity test before initiation of antituberculosis drugs. Data on demographics, chief complaint, history of use of antituberculosis drugs, and chest X-ray radiology were collected. Results: Of 85 subjects, 41.18% (35 subjects) has primary resistance tuberculosis. Primary mono-drug resistance tuberculosis were found in 21.18% (18 subjects) in which resistance in streptomycin was the most frequent (10 subjects (11.8%)). Primary poly- drug resistant tuberculosis were found in 13 subjects (15.27%). Affecting mostly streptomycin and ethambutol (SE) (4.70% / 4 subjects ) Primary MDR-TB were found in 4 subjects (4.71%). Conclusion: The primary resistance rate is high in patients with pulmonary tuberculosis category I, H. Adam Malik Hospital-Medan, Indonesia. So, it is necessary to improve the vigilance and efforts in primary resistance management.
