Medical accidents are generally classified into two types: accidents caused by healthcare staff while providing services and accidents caused by patients’ actions. Among the latter, patient falls remain significant problems. Our research goal is to establish a methodology for preventing patient falls that enables us to identify dangerous situations for each patient and determine concrete countermeasures to prevent dangerous situations based on the result of assessment. In this paper, we first present a structure of risks and countermeasures identified by analyzing actual cases of patient falls in 2 hospitals. Second, we design a logical model, developed by visualizing and structuring thinking processes of expert nurses based on the structure of risks and countermeasures. Third, we develop an implementation system based on the logical model. Finally, we applied our model to an actual hospital to confirm its validity. The logical model and implementation system we developed makes it possible to efficiently and effectively determine countermeasures to prevent falls for each patient. This study is expected to contribute considerable in the efforts to ensure healthcare safety and improvement of the quality of healthcare.
