Isi artikelA 6-year-old boy with type IV osteogenesis imperfecta presented to the emergency department with pain in the right leg after a fall. Plain radiographs revealed an undisplaced, transverse fracture of the middle shaft of the right tibia (Panel A, arrow). The images also revealed the presence of radiographic zebra lines (arrowheads). Radiographic zebra lines are a manifestation of the administration of cyclic bisphosphonate therapy in children before closure of the epiphyseal growth plates. Around the time of bisphosphonate administration bones become dense and more radiographically opaque, and multiple transverse linear opacities are produced across the metaphyses of growing bones. The distance between each line is dependent on the patient's age, the rate of bone growth, and the bisphosphonate dosing regimen. This boy had been receiving intravenous pamidronate every 3 months to mitigate the increased bone fragility that characterizes osteogenesis imperfecta. He wore a cast for 3 months, at which point radiography revealed cortical union (Panel B, arrows), and he had full clinical recovery from the fracture. As expected, the radiographic zebra lines remained visible (Panel B, arrowheads).
