The diagnosis often misleading because of the unusual clinical signs and symptoms even the appropriate surgical management varies between each etiology. Case Presentation Here we present a case of perforation in the second part of the duodenum which has unknown etiology. Damage control surgery was performed successfully using an insertion of a T-tube. Conclusion Perforation of the second part of the duodenal segment is very rare and the cause of the perforation of this patient is unclear, thus making a definite treatment a challenge in which there is no standardized guidelines for each etiology.
