Abstract
Manufacturing is the business with the highest accident rate every year. One type of manufacturing is fabrication. PT. Fajar Gemilang Semesta is a company engaged in the workshop Fabrication Workshop. Risk analysis was conducted at PT. FGS that has not been certified OSHAS 18001, because it has not implemented SMK3 well. This study aims to identify potential hazards and risks that may occur in operations. Hazard identification is done by using Job Hazard Analysis method. The identification results are used to calculate the risk hazard that can be produced by Qualitative Method. Then, the calculation result will be evaluated using Fault Tree Analysis (FTA) and become the base of SMK3 program design to reduce the potential hazard. From the results of risk reversal, it is known that there are 27 potential hazards, where 40.74% (11 risks) are high risk levels; 37.04% (10 risks) is a moderate risk level and 22.22% (6 risks) is a low risk level. Risk control is performed on several main causes, namely: APD, Layout, Machine Speed, Station Ventilation and SWL Sling.
