RSUP. Dr. M. Djamil is an institution dealing in the health sector that provides medical services. An early survey resuult showed that lot of complains that happen focusly caused bt many activities that non value added. The purpose of this research was to investigate any waste sources and the causes of waste and to eliminate of waste, it has an added value. Lean six sigma was a alternative to improve the system. In Define step was known there are Gap (-1,22) between the satisfaction level and the important level with the sigma level less than 3 and DPMO 207.000-393.000 in the Measure step. To analyze step must be done by mapping all detail activities by using Process Activity Mapping (PAM), this would be usable to formulate any costing source and then Big Picture Mapping (BPM) identified the costing reason on the fixing process. Process Activity Mapping (PAM) showed a long term waiting time on the fixing process where it was identified as a waste. Big Picture Mapping (BPM) was known 5 types long term of waiting time waste in help processing the patients. Afterwards, it was used error proofing to minimize the high cost source and error-proofing solution for service was implemented in this step. 5W-1H was used as a tool to give a detail activity planning recommendation. Result 5W-1H are to the helping of patients is the medicines that needed by patients have to send fast, to move the nurse's rooms to the strategic places, to check the stock although the quality of medical instrument, to add the medical worker value and to give training to the nurses. While at the patient's registration is adding the administration of employer, to add the work time of chasier and the worker are always make the patients to pay attention on the registration that have to be completed and to put the procedures that have to be faced by patients at the strategic place.
