Arandomized, controlled, double blind trial was conducted at two Sub-districts ofIndramayu, West Java to evaluate the effect of zinc supplementation on acute diarrhea among children under 3 years of age. The treatment group received zinc in the form of syrup in an average daily dose 4.5-mg elemental zinc/kg body weight/day administered twice a day during the episodes of diarrhea. The control group received the same syrup but without zinc. During 12 months of observation, between April 1995 and March 1996 there were 2410 episodes, but 131 episodes were excluded due to lack of information. Among children in the zinc supplementation group, there was an 11% reduction (95% CI 3 -18%) in the risk of continued diarrhea. In children who had ~ 3 episodes of diarrhea during the 12 months observation period there was a greater reduction in diarrhea duration (RR in the zinc-supplementation group =0.79; 95% CI 0.64 - 0.97). In the zinc-supplementation group, a 22% reduction (95% CI 7 -35%) was observed in the proportion of episodes lasting more than four daysafter enrollment. In children under 3 years of age oral zinc syrup with an average dose 4.5 mg! kgbodyweight/day, which was administered by the mother at home significantly reduced the duration of diarrhea. The use of zinc supplements in the treatment of acute diarrhea showed in significant important reduction in the morbiditYand mortalitYcaused by diarrhea in children in developing countries.
