Abstract
The risk for workplace accidents may occur in all levels of job in a factory, either caused by machines and its operators or work environment. The observation conducted in PT. ATMI Cikarang reveals the fact that the room condition of stamping unit is at risk for workplace accidents due to the placement of products, materials, and scraps which is not neatly arranged so that can fell on the operator, ergonomically unsatisfied removal position, and lifting load exceeds the operator capabilities. This bachelor’s thesis research is carried out to analyze risk factors for workplace accidents, dominant risk factor, and alternative improvement to reduce the risks. Appropriate methods that can be applied are Failure Mode and Effect Analysis ( FMEA) and Fuzzy Logic ( Fuzzy FMEA). The combination of these two methods in its application is able to prevent and/or early examine the risk of accidents, to determine priority risk factor with a given solution gradually. The results show that the risk factor that most probably occur with the highest FRPN value of 886 is the risk factor of workplace accident struck by objects. The proposed improvement alternatives are the occupational health and safety training, making of selves, setting of standard of operational of the arrangement of items, and 5S method which are expected to reduce risk factor for workplace accidents.
