Abstract
Background. Diabetes Mellitus (DM) was and is still a major health problem that is responsible for numerous long term complications and multiple organ dysfunctions. Inflammation process plays a noticeable role in the pathogenesis and development of type 2 DM. Neutrophil to Lymphocyte Ratio (NLR) is a cheap, simple and accessible inflammatory indicator, obtained from leucocyte count. NLR is an inflammatory indicator for bacterial and viral infection, cardiovascular disease and cancer. Systemic inflammation measured by NLR has a significant association with glucose intolerance and chronic diseases.
Objectives. The aim of this study is to understand the role of NLR in differentiating type 2 DM and non-DM.
Methodology. The study was conducted in July 2016 until October 2016 at Atma Jaya Hospital. For this analytical comparative study, cross sectional method was used and the subjects were enrolled with consecutive sampling method. There are 59 patients with type 2 DM and 160 control subjects that participated in this study. NLR was counted from blood smears and patients’ characteristic was obtained from medical records. Independent t test, Mann-Whitney U test and ROC curve analysis were used to analyze the role of NLR.
Results. Median value of NLR in type 2 DM (1,94(0,67 – 10,00)) was higher than non-DM (1,34(1,00 – 1,80) with p < 0,05. NLR was found to have a role in differentiating type 2 DM and non-DM with cut-off point of 1,74 (AUC 81%, sensitivity 61,02%, specificity 99,38%, positive predictive value 97,30% and negative predictive value 87,36%.) With 89,04% of accuracy, value = 1,74 was considered positive for type 2 DM.
Conclusions. NLR have a significant role in differentiating type 2 DM and non-DM. Future study is needed to investigate what is the association between NLR and type 2 DM.
