Patients are frequently misdiagnosed, and experience a chronic course of illness with frequent recurrences. Objective : To determine the course of illness, diagnosis and differential diagnoses, treatment, patient adherence and its contributing factors of bipolar affective disorder. Method : Case study of a patient through psychiatric interviews, physical and mental state examination, and secondary data from corresponding medical records. Results : The patient is diagnosed with bipolar affective disorder, current episode severe depression with psychotic symptoms. Course of illness is chronic progressive with repeated recurrence and psychotic symptoms. Lack of knowledge and insight, side effects, and poor family support causes suboptimal treatment adherence and a chronic recurrent course of illness. Conclusion : An early and appropriate diagnosis, comprehensive treatment which includes the patient’s family from the beginning contributes to the course of illness and quality of life.
