The complete unilateral cleft lip is a congenital deformity on the lip due to failure of embryonic lip fusion. Primary correction of the cleft deformity should restore the functional balance and correct the obvious anatomical derangement. Clinical appearance of individuals with the defect some¬time can not be hide for their specific face and lip pattern. A patient, twenty four weeks (six months) old infant, in his first visit to the Dr. Sardjito Hospital presented complete unilateral cleft lip and nose deformity. A unilateral cleft lip surgery was carried out on the Millard's technique concurent with modified V- Y flap rhinoplasty method. Evaluation on surgery result was completed using Cleft Lip Component Symmetry Index (CLCSI). The result of the treatment presents aesthetically well adjusted and mentally supportive face. There are quite fast wound healing process, slight appearance of scars on the lips, and normal lip muscles tension.
