Isi artikelBackground: Ovarian Stimulation aimed to retrieve good quality ovum on Ovum Pick Up (OPU), however ovum still varies in maturity. Variation in ovarian stimulation protocol at Permata Hati Clinic yield in 76% M II ovum (unpublished data), the rest are premature and ovum with unoptimal cytoplasmic. In Vitro Maturation (IVM) is a technology in the field of human reproductive that allowed ovum to have a possibility to be matured in vitro. Objective: The objective of this study is to observe immature ovum achieving M II by in vitro maturation, fertilization, and embryo development. Methods: All of Intra Cytoplasm Sperm Injection (ICSI) cycles administered at Permata Hati Clinic from January 2007 until August 2008. Patients having ovarian stimulation: Long GnRHa, Antagonist, and simple stimulation were quoted in the study. Ovum maturity status were assessed 3-6 hours post OPU, immature ovum were cultured in standard media for 24 hours. Ovum achieved M II, fertilized by ICSI, and assessed their fertilization, embryo development until day 3. Results and Conclusion: This study reports that Long GnRHa produces better M II as compared to those Antagonist and simple stimulation (p=0.00) after 24 hours culture in vitro. In the group of Long GnRHa, fertilization is different in proportion between 3-6 hours incubation and 24 hours IVM (p=0.03). Fertilization of ovum which is cultured for 24 hours are not different neither in group Long GnRHa, antagonist, nor simple stimulation (p=0.92). Embryo quality of ovum incubated 3-6 hours are not different compared to embryo quality of ovum 24 hours IVM (p>0.05). The embryo quality are not different in all 3 group of ovarian stimulation protocol, when the ovum need 24 hours IVM (p=0.79). This study explained that ovum have not yet achieve M II on OPU, could be cultured for 24 hours in standard culture media without additional hormonal regiments in vitro, to develop to M II. Fertilization could be achieved by ICSI and good quality embryos are observed. Long GnRHa is proved to produce better ovum maturation of 24 hours IVM. However, fertilization rate and embryo quality are not different in all 3 groups of ovarian stimulation protocols.
