Abstract
Introduction: Of the 15 million patients affected by stroke worldwide yearly, the World Health Organization reports almost a third of these patients die, and another third are permanently disabled. Leukocyte count has been shown to be associated with an increased risk of stroke. A study showed that leukocyte count was predictive of first ischemic stroke. Some studies had found that increased leukocyte count in the first day of acute ischemic stroke is a prognostic factor of disease progression. There were also some studies that did not find an association between the admission leukocyte count and clinical outcome.
Objective: The purpose of this research is to find out the relationship between total leukocyte count and neurological deficit on admission in patients with acute ischemic stroke at Atma Jaya Hospital in 2013/2014.
Methods: The cross sectional study was implemented through data collections that have been conducted in Atma Jaya Hospital using medical records of acute ischemic stroke patients in 2013/2014. The records contain patient’s characteristic, total leukocyte count, and initial NIHSS, which then being analyzed with Chi-Square test and Mann-Whitney U test.
Results: From total 67 patients, the majority (64.2%) of patients have normal total leukocyte count, while the other (35.8%) patients have leukocytosis. Average total leukocyte count for all patients on admission is 10.02 (±2.54) x 103/µl. Average initial NIHSS for all patients on admission is 7.98 (±6.00). Chi-Square test revealed a significant relationship between mild-moderate and severe initial NIHSS and total leukocyte count (p=0.021). The test also revealed a significant relationship between mild and moderate-severe initial NIHSS (p=0.001).
Conclusion: This research showed a significant relationship between total leukocyte count and neurological deficit on admission in acute ischemic stroke patients at Atma Jaya Hospital in 2013/2014.
