Continuity of care ensures a smooth transition between hospital care and home care which is more commonly referred to as dicharge planning. An effective discharge planning will pro¬vide guidelines and coordination for continuity of care including teaching all the important aspect based on the needs of the client, preparation of maternal clients for parenting and her independent (Bobak, 2005). The purpose of the study to identify the teaching needs of the postpartum mothers. This research is a descriptive reasearch with the numbers of sample 88 respondents. It was conducted in a maternity ward, of a Sumedang General Hospital for the period of December 2006 until February 2007. Data were collected through survey. Analysis univariate and frequency distribution. The re¬search result shown the characteristic of the respondents includes: average age 25.5 years old age, primiparity 61.36% and mostly level education is high school (40.91%). 47.16% responden want to know care of mother while postpartum. Teaching who respondent want to know mostly are postpartum exercise (84.09%), care of episiotomy (75%),risk factor for postpartum complication (65.91%). The least respondent want to know are nutrition while postpartum(31.82%), sibling (31%) and rest time (31%).
