All the major rheumatology textbooks tell us that the chief determinants of the frequency of systemic lupus erythematosus (SLE) are sex and ethnic origin. The correlation between ethnicity and the epidemiology of SLE is clearly complex. For example, SLE is apparently common in China’ and among Chinese people in Hawaii,~ whereas in West Malaysia the prevalence of SLE is just 1.5 times that in other ethnic groups, and in San Francisco Chinese people have the same frequency of lupus as the local white population.4 This editorial examines the varying frequency of lupus in people of African origin in Africa, the Caribbean, the USA and Europe and discusses the possible explanations for the observed differences.
