Down syndrome accounts for one third of all cases of moderatesevere mental retardation. Recent advances have improved maternal screening and increased our understanding of the underlying pathophysiology. Understanding of the spectrum of medical complications of this disorder is also improved, including atlantoaxial subluxation, hypothyroidism, and Alzheimer disease. Life span and functional outcome have improved with better medical care and improved educational, residential, and employment opportunities. A critical feature is the need for anticipatory management in which the pediatrician plays the crucial role.
