Isi artikelSince the early days of neonatal intensive care, the use of analgesia and sedation for the neonate has been controversial. As recently as the 1980s, the use of analgesia for postoperative pain in the neonate was uncommon. This practice reflected the difficulty in assessing pain in nonverbal patients and the assumption that central nervous system immaturity blunts the perception of pain in neonates. It is now clear that even extremely low birth weight infants have hormonal and metabolic responses to painful procedures and that these responses can be ameliorated by administration of anesthetic or analgesic agents. Moreover, the use of such agents to reduce pain during and after neonatal surgery has been reported to reduce postoperative complications.These findings strongly suggest that neonates do experience pain and that it may be important to treat mild to moderate pain, as well as severe pain, in neonates. These observations and concern for providing compassionate care have prompted routine use of postoperative analgesia. The safety and effectiveness of the different regimens used for postoperative analgesia in neonates have not been well studied.
