Congenital heart disease in the most common, major birth defect, responsible for much mortality and morbidity infants and children. Careful history taking and thorough physical examination enable the assesment of the signiffcance of cardiac symptoms, the functional status of the patient, and the possible undertying cardiac defect in some cases. Diagnostic workup should be apploached in a systematic manner, starting from careful clinical evaluation followed by simple laboratory investigations such as electrocardiogram, chest roentgenogram, haemoglobin concentration and blood gases, before more so phisticated investigations are ordered. A combined electrocardiogram and chest roentgenogram approach gives a fair idea of the differential diagnoses of cyanotic congenital heart disease which is generally considered a difficult diagnostic problem by general physicians. Further investigation by echocardiography often provides adequate visualisation of the anatomical defect and its functional significance so that surgical therapy may procced without invasive study. Before contemplating invasive diagnostic procedures, it is important to balance the risk and the accuracy of the information obtained. Cardiac catheterisation is now generally reserved for patients whose operability depends upon critical haemodynamic assessment, for those whose cardiac anatomy is not adequately demonstrated by echocardiography, and for those who undergo a therapeutic pncedure such as balloon dilatation of the stenotic pulmonary valve combined with manometric and cineangiographic studies in the same sitting.
