Cardiovascular disease is the leading cause of death in the developed world, and it is poised to become the most widespread health problem worldwide. Atherosclerosis is the major cause of cardiovascular disease. An elevated level of plasma low-density lipoprotein (LDL) cholesterol is the sine qua non for the development of atherosclerosis.1 People with lifelong low LDL cholesterol levels are at very low risk for cardiovascular disease, yet the dramatic reduction of LDL cholesterol through the use of statin drugs over several years reduces the risk of heart attack by only 40%, largely because the disease is already established when treatment . . .
