Being recognized as an excellent home care and hospice organization has always been very important to us at the Visiting Nurse Association (VNA) of Maryland. As the time for our triennial accreditation survey with the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) grew near, so did our anxieties, because we knew that additional requirements were expected this time. Was ORYX really the way to go to measure the quality of care in our setting? With home care finances being challenged with the Interim Payment System (IPS) and the forthcoming Perspective Payment System (PPS), could we afford this survey? With more survey standards being released, how relevant were they to the daily practice of home care and hospice? From 2000 to 2001, these issues prompted us to explore alternatives to JCAHO accreditation.
